I, the parent/guardian, hereby grant permission for my child to attend the camp. I understand that there is always a possibility that my child may sustain a physical illness or injury while at camp. In the event of such illness or injury, I authorize the camp staff and representatives to provide first aid and to contact me immediately to determine whether my child should be referred to a medical treatment facility (hospital, clinic, etc.) or receive further medical attention if necessary.
I acknowledge and understand that I will be responsible for any costs incurred for medical treatment related to any illness or injury my child may sustain during the camp. I hereby release Glad Tidings Church, including its camp staff and representatives, from any claims arising from personal illness or injury sustained by my child during the camp.
I further acknowledge and understand that my child is expected to follow all camp rules and regulations and will be held responsible for any failure to do so.
I also give permission for my child to be photographed and/or video recorded while attending the camp. Photos and videos may be used in slideshows and posted on the church's website and other church-related media for promotional or informational purposes.
